Has your local government or municipality asked you to apply for a permit or license, submit a request or survey, or fill out a different type of form submission workflow on ClearGov's Online Forms (ClearForms) platform? If so, you are an applicant and this article is for you! There are a few important steps to filling out a form submission. Select the link below to jump directly to that section of the article.
Start your form submission
To get started on a form submission (permit, license application, request, survey, etc.), first find the link on your government or municipality's website. Select the appropriate link, and you'll land on a page that looks something like this (see screenshot below).
Select the Start button to proceed to the first step of the submission.
Note: As you scroll down the page, the Start button will persist, so you can review all the steps and not have to scroll back up to begin.
On the Start Page, you can see each step of the form including what type of activity the step requires, and whose responsibility the step is.
For example, a step you are responsible for might be providing contact information or making a payment, whereas internal review or approval steps will be handled by an administrator of the government entity whose form you are submitting.
If the step is your responsibility, it will have a bold Your responsibility label in the Responsibility section. The entity that created the form will complete the other steps.
Create your ClearForms login
Once you select the Start button, you'll be prompted to create a ClearForms account.
This allows you to access your applications at any time. It only takes a few seconds to create an account, and all that's required is your email address and a password.
A few things to know about your ClearForms account:
- After you create an account and start one or more submissions, you'll be able to sign back into https://go.citygrows.com any time to check on the status of any application, license, or permit you've filed.
- Provided you sign in with the same email, you can access all the ClearForms submissions you've started in one place.

In the event you forget your password, you can always select the Forgot password link on the login page. Instructions on how to reset your password will be sent to the email address in our system.
Note: be sure to check your Spam or Junk folder(s) if you do not see the forgot password email after a few minutes.
Now that you have created an account, you’ll proceed to the first step in the submission.
Sign a form submission
Some form submissions will require you to sign your name before submitting and moving on to the next step.
When prompted for a signature, follow these steps:
- Type your name into the Name field
- Select the Sign button. Your signature will appear.
- Select the Submit button to save your signature.
