What are Forms?
Forms allow you to collect structured information and feedback directly from your community. When creating a form, you can choose from a variety of response types to best capture the information you need from your community.
Adding a Form
Follow the steps below to add a form to your Engagement Website.
- Navigate to the Design Tab under the Content & Design heading and select a page.
- Add a Form Widget to the location on the page where you would like to post your Form.
- Indicate whether you would like to create a new form or select an existing form from the dropdown list.
- If creating a new form, enter a form name and select a Form Type: General Submission or Letter of Submission. This form type will determine the category of the form.
- Select the Add Form Content button and the type of form you are creating from the drop-down menu,
- Select Save to add the Widget to your Engagement Website.
Types of Form Content
Each Form Content type serves a specific purpose, allowing you to customize your form to fit your engagement goals. The following types are available in the Add Form Content dropdown menu:
- Short Response: A single-line input field ideal for collecting brief information, such as a name or contact detail.
- Long Response: A multi-line input field designed for collecting more detailed information, such as open-ended feedback or comments.
- Checkboxes: Presents multiple options where users can select all that apply, ideal for questions with more than one possible answer.
- Single Select: Presents a list of options where only one can be selected, ideal for questions with a single definitive answer.
- Ranked Choice: Presents multiple options that users can rank in order of preference, ideal for understanding community priorities.
- Description: A rich text editor for displaying long-form text without an input field, ideal for providing context or instructions within the form.
- Divider: A horizontal line used to visually separate different sections of a form, ideal for organizing longer forms into distinct sections.
- Image: Allows you to upload and display an image within the form to provide visual context or support form content.
Viewing and Managing Form Responses
The Form Management section allows you to view and export the responses submitted by your community.
- Select Form Responses under the Feedback section in the navigation sidebar.
- Select All, Active, or Archived to filter forms by category.
- To download form responses as a CSV or PDF, select the action dots on the header row of the desired form.
- To view the details of a specific response, select the action dots for that individual response.
